Boonville, IN. — Reba Bernice Polk, 95, of Boonville, Indiana passed away on Sunday, May 2, 2021 at Woodmont Health Campus in Boonville.

Reba was born in Warrick County, Indiana on October 21, 1925 to the late Alex and Katherine (Pate) Bolton.

She was a devoted Christian and was a member of the Tennyson General Baptist Church. She was a big Cardinals fan.

She is preceded in death by her parents, husband, William H. Polk; son, Jerry Lane Polk; siblings, Irvin Bolton, Nolan Bolton, Royce Bolton, Nera Grant, Lena Kendall, Lois Walker.

Reba is survived by her children, Michael R. Polk and his wife, Judy of Boonville, IN; Karen Collins of Boonville, IN; Kathy Adams of Boonville, IN; grandchildren, Deniese Collins, Rachel Long and her husband, Ken; Lisa Polk, Shari Buhmeier, Jeff Polk and his wife, Carla; Tara Fisher and her husband, Bob; Shelli Adams, Jennifer Adams; 10 great-grandchildren; several nieces and nephews.

Services will be 11 a.m. on Thursday, May 6, 2021 at Koehler Funeral Home in Boonville, Indiana with Pastor Bill Tennyson officiating. Burial will be at Polk Cemetery in Tennyson, Indiana.

Visitation will be from 9 a.m. until 11 a.m. on Thursday, May 6, 2021 at the funeral home.
